Traditionally, an extraction site is allowed to heal for several months before an implant is placed (first stage). Once the implant is inserted, the tissues are sutured over the implant. The implant is exposed to the oral cavity 3-6 months later (2nd stage), after the “osseointegration” period. This traditional approach results in high predictability, at the cost of not having an implant available immediately for support. Today, this approach is still essential when primary stability of the implant at time of insertion can not be accomplished, or when grafting procedures are necessary. To significantly increase the success of these bone grafting procedures, the graft should be protected by a membrane, and the membrane should be submerged under the tissue.
